STAYING CONNECTED

A Faith in Action program for seniors. Interfaith volunteer caregivers are trained and supervised to help the frail elderly remain independent in their homes. Free services include

  •  Transportation for health care appointments
  •  Transportation for essential errands including grocery shopping and banking
  •  Friendly visits
  •  Reassuring telephone calls
  •  Advocacy

 COUNSELING

Individual, marital, and family counseling to help clients explore constructive ways to cope with challenges.

 CASE MANAGEMENT

  • Provides concrete resources to individuals and families, such as food, emergency funds, and bus passes
  • Helps to connect clients to community agencies, such as: mental health services, disability related services, etc

 GUARDIAN SERVICES

  • Healthcare advocacy and decision making
  • Management of estate and financial affairs.

 

  

GERIATRIC CARE MANAGEMENT

  • Older adult assessments
  • Development of care plan
  • Exploration of options regarding in-home support services, assisted living or nursing home placement, and availability of community resources
  • Personal affairs management
  • Member of National Association of Geriatric Care Managers
JEWISH OUTREACH
  • Bikur Cholim program of hospital, assisted living and nursing home visits. For more on this mitzvah, check out this page.
  • Kosher food pantry for those in need.
  • Home visits

  • Programs for senior adults in collaboration with York County Area Agency on Aging and AARP
